Re-Scheduling and Missed Appointments*
Fort Wayne Dermatology Consultant (FWDC) uses a service to send appointment reminder notifications to our patients prior to their appointment. This notification will include the date and the time. We perform these calls as a courtesy to our patients. This allows the patient the opportunity to cancel the appointment should the date or time not work for you or if the appointment is no longer necessary. Cancelling in advance, allows us to schedule another patient during that time. Recognizing that everyone’s time is valuable, and appointment times are limited, we kindly ask that you provide 24 hours’ notice by calling us at 260-436-8000, if you are unable to keep your appointment.

Sometimes, we have patients who routinely do not notify us that they need to reschedule or cancel. Because of that, FWDC has a no-show policy. The policy is as follows:

  • After the first missed appointment (no show or cancel with less than a 24 hours’ notice), the patient will be notified about the missed appointment by text, voice message and/or email.
  • After the second missed appointment or cancellation with less than a 24 hours’ notice in a 12-month timeframe, the patient shall receive a letter from the practice notifying the patient of the second missed appointment and a reminder of the No Show policy. The patient will be required to provide a $100 deposit when rescheduling for the next 12 months.  The deposit will be applied to any balance due after the appointment or refunded if needed.  If the patient is a no show or cancels with less than a 24 hours’ notice at any time during the next 12 months, the deposit will not be refunded, and the patient will be discharged from the practice.
  • If a patient is a no show or cancels with less than a 24 hours’ notice for a New Patient or Surgery appointment, the patient will be required to provide a $100 deposit when rescheduling. The deposit will be applied to any balance due after the appointment or refunded if needed.  If the patient is a no show or cancels with less than a 24 hours’ notice for a New Patient or Surgery appointment for a second time, the deposit will not be refunded, and the patient will not be allowed to reschedule.

These are general guidelines and mitigating factors need to be considered before making a final decision. Mitigating factors could be the length in time between missed appointments or why appointments were missed.  In certain circumstances, an appointment cancelled with less than 24-hour notice may also be included in this policy.

Cancellation Policies for Aesthetic Appointments

Your appointments are very important to us and reserved especially for you. We do understand that sometimes schedule adjustments are necessary; therefore, to cancel your appointment, please call our office 24 hours prior to your appointment at 260-469-1400. Please review our Aesthetics Cancellation Policies below.

In effort to reduce the number of no-shows and last-minute cancellations, we have implemented an updated Appointment Cancellation Policy that went into effect on January 1, 2022.

2023 Cancellation Policies

Please read and be familiar with our Cancellation Policies prior to scheduling appointments.

We kindly request a 24-hour advanced notice for cancellations and reschedules. 
Failure to cancel or reschedule an appointment with a 24-hour notice will result in forfeiting booking deposit.

Mobile Number on File: We kindly ask all patients to provide us with a mobile number so we have a means to contact you and for you to receive appointment reminders.

Aesthetic Appointments: A $100 deposit is required at time of booking. Deposit will be applied to treatment received day of service.

VIP Members: No deposits are required for VIP Members.

Consultation Fees: We will charge a $100 fee for all consultation appointments (CoolSculpting and CoolTone services are exempt). The $100 fee will be applied to any product or service purchased. No-shows and last minute cancellations will be charged and will forfeit the $100 fee.

**Patient No-Shows and Cancellations without 24-hours notice: Deposits made at time of booking will be charged and will forfeit the deposit made. Last minute cancellations apply to these charges.

PLEASE NOTE:
If patient has purchased a treatment package, a credit card is not required to be on file. However, if patient is a no-show or does not cancel or reschedule
without a 24-hour notice, patient will lose $100 from their package payment.

CoolSculpting ELITE and CoolTone Products are exempt from booking deposits and consultation fee policies. However, if patient is 15 minutes or more late for scheduled appointment, we may have to condense your treatment or reschedule it to avoid inconveniencing patients scheduled after you. Please refer to your signed contract pertaining to additional exclusions that apply.
